RN Case Manager/Admissions RN – Hospice (RN - MO Licensure Required)

  • As a RN Case Manager/Admissions RN – you will be responsible for ensuring the delivery of quality hospice services as well as the coordination of services that reflect the philosophy and standards of hospice nursing care of assigned patients. 
  • Assess patient conditions and family / caregiver's conditions accurately and completely during each admission or visit 
  • Identify and provide for patient and family's educational needs.
  • Provide patient care per physician orders and Agency policy.
  • Complete all required patient and Agency documentation in an accurate, thorough, legible and timely fashion to reflect a clear picture of care provided.
  • Transcribe, implement and evaluate new physician orders accurately.
  • Work collaboratively with patients, care providers, family members and IDT 
  • Communicate patient's issues and concerns with appropriate health care professional, hospice interdisciplinary team and / or Clinical Manager in a timely and accurate manner.


LPN – Hospice Routine Visit Nurse (LPN – MO Licensure Required)

  • Under the direction of a registered nurse to provide quality nursing care to patients.
  • Assist with collection of assessment data and provides input to the planning of care. 
  • Implement plan of care within the scope of licensure. 
  • Perform all duties in a manner which promotes team concept and reflects the mission and philosophy.


Licensed Practical Nurse (LPN)

  • Must have good standing license (LPN) in the state
  • Must have at least 6 months of hands on nursing experience (willing to train new grads) 
  • Current CPR certification/Current TB Test
  • Must be willing to provide in home nursing care young adults, children and adults
  • One to one patient care in a home environment
  • Execution of physicians prescribed plan of care and compliant documentation of care provided in system of record
  • Administration of prescribed medication, treatments and therapies
  • Patient assessments, coordination of care
  • Health promotion, teaching, and training of family members
