During the course of a person's illness, there are times when the level of care may change to provide more care and support. Hospice offers four levels of care.

Routine hospice care

The most common type of hospice care, routine care is offered in assisted living and independent living communities, boarding homes, long-term care facility or in the home, if possible.

General inpatient care

General inpatient hospice care is an option for patients whose pain and symptom management are not being managed properly in a home environment. This care is in an acute care hospital or other settings were intensive nursing and other support is available outside of the home. Care is provided by the facility staff and the Deer Valley Hospice Care team.

Continuous care

Continuous Care supports the patient and their caregivers through brief moments of crisis. This care is typically provided 8 to 24 hours a day, primarily by an LPN or RN in the patients home.  Home health aide and homemaker services may be used to help cover the care needs.

Respite care

Respite care is short-term inpatient care that relieves the primary caregiver.  Respite care is for 5 consecutive days, provided in a skilled nursing facility (SNF). This option is often used so the primary caregivers can have the opportunity to get away or take care of important things, such as traveling for business or handling their own health care needs.
